  Second Opinion Requirements

If you have had an MRI, CT, MRI or CT with Contrast / Injection, MRI or CT Arthrogram or CartiGram for an orthopaedic or sports medicine condition or injury and wish to have a NationalRad second opinion, we will need the following items to provide you with an accurate second opinion:

  • A CD with the DICOM image files* from your diagnostic image study. If you do not already have this, you can get it from the imaging center or hospital that performed your test.
  • Your original radiology report, which you can request from the imaging center or hospital along with the CD.
  • Any prior diagnostic imaging studies, such as MRI, CT, X-ray and Ultrasound, that are relevant to your current case for which you are seeking a second opinion.
  • Any prior operative notes and / or pathology reports that are relevant to your case, if applicable.

* PLEASE NOTE: We only accept DICOM image files through our ImageShare system. DICOM is a specific industry standard for radiology, and the imaging center or hospital where you had your tests performed should be able to provide images in this format. However, some software will export images in non-DICOM format. If you have images that are in non-DICOM format or you are having difficulty uploading your image files from your CD, please call us toll-free at 877.938.2778 or email us at consult@nationalrad.com so we may assist you directly.

  Second Opinion Questions & Additional Information

In addition to the requirements listed above, when you submit your case through our ImageShare system, you will be asked to provide information that will help our radiologists understand your relevant medical history, including:

  • Symptoms you are experiencing.
  • How and when your symptoms began.
  • Any prior treatment you have had, if any.
  • Any prior imaging studies you have had, if any.
  • Any additional medical and surgical history that is relevant to your second opinion case.

Depending upon your answers, we may ask you additional questions about your condition or injury to fully understand your condition or injury.

Please answer all questions completely and to the best of your ability to help our radiologists fully understand the nature of your case. When it comes to a second opinion, the more relevant information you can provide, the better we can assist you.

  Restrictions

For U.S. Patients

Due to state laws governing the practice of medicine in the following states, our radiologists may not be able to provide you with a second opinion if you are a resident of or have a current mailing address in any of the following U.S. states:

  • Iowa
  • Louisiana
  • Maine
  • Michigan
  • North Dakota

Investigational Studies

  How to Submit Required Documents

If you have digital or electronic copies of your original radiology report and / or your prior surgical reports, you will be able to upload them through our secure ImageShare system when creating your case.

If you have hard copies of any of your reports, please fax them to us toll free at 888.416.4853. Click here to download our Second Opinion Fax Cover Sheet.
